California Children Services and CARE Parent Network together planned and implemented the Medical Home Project, funded by the California HealthCare Foundation. Our project was one of seven projects located throughout the state which were guided by a Statewide Coalition. The purpose was to work with primary care providers, parents, and other service providers to create a medical home for children with special health care needs. Children with special health care needs include children with chronic physical, developmental, behavioral, or emotional conditions who require health and related services of a type and amount beyond that generally required by children. Consequences of their conditions include increased visits to hospital emergency rooms and missed days of school, as well as detrimental consequences for parents including inability to work. Coordinating the needed care and facilitating access to resources improves outcomes for children and quality of life for the family. The activities undertaken through our Medical Home Project have included strategic planning through the convening of providers, agencies, and parents; completion of surveys to collect relevant data; documentation of unmet family needs; outreach to families, related service providers, and primary care providers; training for families and providers; and evaluation of project activities. |
